Muscular Strains-Grades, Symptoms and Treatment

In Dr. Koshes’ online CEU massage therapist course, “Clinical Neurology and Orthopedics for the Massage Therapist,” he reviews commonly diagnosed musculoskeletal conditions with protocols for treatment. He teaches Florida LMT’s that muscular strains can be divided into three grades. With a Grade I strain, the injury does not stop the person’s activity and the muscle fibers are left intact. The patient will experience mild pain with slight swelling or muscle tightness. In a Grade II strain, the injury does stop the patient’s activity. There is an incomplete tearing of the muscle fibers which will produce a partial loss of muscle strength. The patient will experience moderate pain with swelling and a partial disability of function. In a Grade III strain, there is a complete tearing of the muscle fibers causing a complete loss of muscle strength. The patient will experience severe pain with swelling and/or discoloration with a total disability of function.

This LMT CEU class also covers treatment, which initially should include “PRICES” which stands for; protection of the injury, rest of the injury, ice, compression, elevation and support for the injury. Treatment of this type of injury could take 4-6 weeks for Grades I and II with surgery generally recommended for a Grade III strain. In-office care protocol could include massage treatment, acupuncture, physical therapy, chiropractic, mild support bracing and/or exercise therapy.
