Arthritis-Types,
Symptoms and Treatment
In Dr. Koshes’ continuing education courses
for massage therapists “he reviews commonly diagnosed musculoskeletal
conditions with protocols for treatment. In his class he discusses
two major classifications of arthritis; Rheumatoid Arthritis/Rheumatoid
Arthritis Variants and Osteoarthritis.
Rheumatoid Arthritis/Rheumatoid Arthritis Variants
include Rheumatoid Arthritis, Scleroderma, Sjogrens Syndrome,
Ankylosing Spondylitis and Systemic Lupus Erythematosis (SLE).
These types of arthritic conditions are thought to be autoimmune
in nature producing a bodywise destruction of their own tissues,
i.e.., cartilage and soft tissues. The patients can be young or
elderly. Symptoms can include migratory joint pain that will involve
multiple joints. These joints are usually warm to touch and swollen.
These types of arthritic conditions can cause extreme joint deformities
in the later stages.
Osteoarthritis is also referred to as degenerative
arthritis, arthritis of wear and tear and/or old age arthritis.
The patient is usually elderly and will present with one joint
involvement, most commonly in a weight-bearing joint. The patient’s
history will most often reveal an earlier trauma to that joint
which has resulted in the excessive wearing changes within the
joint. Symptoms can include pain/ache/stiffness usually involving
one of the weight bearing joints. These symptoms are usually worse
in the morning hours and ease as the day progresses. The joints
involved are normally not warm to touch and deformities as less
severe than with the rheumatoid type of arthritis.
This massage therapy certification teaches that treatment initially
for degenerative arthritis should include “PRICES” which stands
for; protection of the injury, rest of the injury, ice, compression,
elevation and support for the injury. For Rheumatoid arthritis
heat application even in the initial stages is used rather than
ice. In-office care protocol for both classifications of arthritis
could include massage treatment, acupuncture, physical therapy,
chiropractic, mild support bracing and/or exercise therapy. When
joint destruction becomes excessive and the patient no longer
responds to conservative care a referral for medical evaluation
is appropriate.
